Flat-rate or bonus-category cash back — which is better?

Flat-rate cards (around 2% on everything) win for simplicity and unpredictable spending. Bonus-category cards pay more on groceries, gas, or dining but require attention and sometimes quarterly activation. Many people do best with one of each.

Does carrying a balance cancel out cash back?

Almost always, yes. Interest charges dwarf rewards — a 20%+ APR will erase a 2% reward many times over. Cash-back cards only make sense if you pay the statement balance in full every month.
